At A Healthy Smile Dentistry, our staff is committed to providing our patients with a comfortable environment to receive quality dental care. For your convenience, we’ve compiled all of the patient information and forms you might need prior to your appointment with us on this page.
Making an appointment with our dentist is easy! Simply give us a call at 973-377-2222 or fill out our online form and one of our friendly team members will assist you. If you need to reschedule or cancel an appointment, please call us 24 hours in advance to give us notice so that we can accommodate other patient’s requests.
Our friendly staff will greet you for your first visit with us in the lobby area. We will provide you with new patient paperwork such as a health questionnaire, insurance information request, and your contact information. In addition, we will discuss our patient privacy policy and give you any information you might need to familiarize yourself with our office. If you would prefer to complete your forms prior to your appointment, you can find them here.
Your initial examination with us will typically take an hour to complete. Our expert staff will ask you a series of questions about your oral hygiene routine and listen to any concerns you might have. We believe in educating our patients to empower them to make good choices to improve and maintain their oral health. Following our questionnaire, we will demonstrate how to properly care for your teeth and give you any suggestions you might need. Next, our dental hygienist will clean your teeth. Lastly, Dr. Gohard will complete your exam and recommend any dental treatments that are needed.
We want our patients to feel comfortable during their visit which is why we try to minimize pain as much as possible. If you are undergoing a procedure and begin to feel pain, please let us know. If there is anything we can do to help you feel more comfortable during your visit with us, whether that be to bring you a pillow, a blanket, or soothing music to listen to, please let us know. We want all of our patients to stay relaxed and comfortable during each visit.
We understand that everyone has a different financial situation which is why we do our best to offer affordable dental care in Florham Park. We participate with all PPO insurances such as Delta, MetLife, Aetna, Horizon, Blue Cross and Blue Shield, Cigna, United, Emblems, Concordia, etc. We also accept cash, check, Visa, MasterCard as well as interest-free credit with CareCredit.
